COVID-19 Vaccinations
AstraZeneca vaccine is available for all patients aged 18 and over.
Pfizer vaccine is available for all patients aged 12 and over.
Children’s Pfizer vaccine is available for all patients aged 5 to 11 (inclusive).
ALL patients must complete a vaccination consent form appropriate for the vaccine which is being given to them - please see below (if you are unable to print and complete this prior to attending your appointment, please arrive a few minutes earlier so that it can be done at the practice)

New Patients
Patients new to our practice are welcome to make appointments for these vaccinations – we have clinic times available during the week which are available to all.
Please complete the first page of our New Patient form (found on our FAQ page on this website)
New patients are requested to provide an up-to-date Health Summary from their usual treating doctor - please bring this to your first appointment/vaccination.
Please call one of our sites to book in for your appointment. Please note that there must be a minimum period of two weeks between having this vaccine and any other vaccine (e.g. flu).

Do you have flu symptoms?
NSW Health recommends that anyone with symptoms gets tested at a COVID-19 clinic.
Symptoms include fever, cough, sore/scratchy throat, shortness of breath, loss of taste or smell, or unexplained fever. You should self-isolate until you receive your test results.
General COVID-19 Information
With the exception of workers in high-risk clinical settings, there is no requirement for testing asymptomatic people before returning to work.
